Information for New Patients & Expectant Parents

Peds Care, P.C. is here to provide your child with the high-quality care, education and support necessary to achieve their optimum level of health and well-being. Through disease prevention, proper nutrition, fitness regimens, and regular care, we are able to help parents raise healthy and well-adjusted children.


Appointments

We are currently accepting new patients. Please call our office at 706.278.6628 to schedule an appointment. If, for some reason, your child is unable to keep their scheduled appointment time, call us with 24-hours notice to reschedule.


What to Bring to your First Appointment

In order to provide prompt treatment to your child, please be sure to bring the following when you come in for their initial visit:

Photo Identification
Any medical records pertaining to your visit
Immunization Records
Insurance Information/Card

Please arrive 15 minutes before the scheduled appointment time to fill out necessary paperwork. If you would like to complete the paperwork prior to your appointment, you may print and fill out the appropriate forms found below.


Insurance & Payment

We accept most insurance plans. Our office staff will file any necessary claims and work to get you the maximum amount of benefits allowed by your plan. Payment is required at the time services are rendered, unless other arrangements have been made. For your convenience, we accept cash, Visa, MasterCard, and Discover.

Below you will find our new patient forms. Feel free to print and complete the forms before your child’s first visit with us. While this is not a requirement, it will save you and our staff time when you bring your child in for their appointment.



Expectant Parents


Peds Care, P.C. understands what an exciting time it is when you’re expecting! You probably have many questions about the health of your unborn child and are starting to prepare for their arrival. There are many things you can do to prepare for your the birth of your child, including deciding what health care provider to use. At Peds Care, P.C., we will always put your child’s needs and best interests first.

We suggest new parents schedule a prenatal visit at our office. This will allow you to become acquainted with our facility and staff. Though we suggest it, a prenatal visit is NOT required before your child is born. If you decide that you would like one of our experienced physicians at Peds Care, P.C. to be your baby’s doctor, let your obstetrician know, as well as the hospital staff when you get to the hospital to give birth. The hospital will then inform us when your baby has been born.
